Terms & Conditions

Hockey Victoria (HV) Registration covers an individual membership for the calendar year and includes: Hockey Australia(HA) levy; Hockey Victoria levy; and personal accident insurance. Registration is governed by the Hockey Victoria terms and conditions, and polies and codes.

Powerhouse & St Kilda Districts Hockey Club (PHSTKHC) memberships are governed by the PHSTK polices and codes. Refer Policies page for details.

TERMS AND CONDITIONS:
Whilst PHSTKHC endeavors to provide a safe environment for its players, officials, volunteers and spectators every sport or recreational activity has its own risks. In participating in PHSTKHC events and activities, participants understand and acknowledge the following:

• You acknowledge that PHSTKHC is not responsible for payment of any personal injuries incurred.
• You accept the inherent risks of injury associated with hockey and other PHSTKHC events and activities, and agree to assume such a risk.
• You acknowledge that PHSTKHC has no responsibility or liability whatsoever, including for negligence, in relation to any injury, illness or accident sustained whilst participating in training, match play, and PHSTKHC events and other activities. You agree to indemnify PHSTKHC and any PHSTKHC official, coach or organisers, in respect of all actions, claims, demands, losses and expenses incurred or suffered by PHSTKHC whatsoever in respect thereof.
• You authorise the PHSTKHC coaches, officials or club organisers to seek and provide on your behalf, and at your expense for any medical treatment that is deemed necessary including any anesthetic administered by a legally qualified person.
